Format Instructions for Grant Application/Renewal

1. Use grant application form provided.
2. Submit original with 12 copies. Do not use script.
3. Budget/Budget justification: Complete budget request on page 3 of application form and provide justification for all items
sought. Please note that Association for Research of Childhood Cancer does not support institutional overhead, personnel fringe benefits or overhead, travel costs, books or journal subscriptions or general
office/clerical expenditures.
4. Biographical Sketch: Attach relevant curriculum vitae for each investigator.
5. Other support: Provide title listing of all other research projects both funded and pending for all investigators.
Cite granting agencies and amount of award funded or sought.
6. Research Plan: Attach a concise research plan (NOT to exceed 5 pages) to include:
7. Citations: Provide pertinent references only.
8. Letter of Support: Attach a letter of support signed by a responsible officer of each parent institution delineating their approval
and support of the investigators and the project.
9. Renewal applications (Category II):
Awards should include a progress report in place of the research plan, an updated CV, and any additional research awards finalized or sought since the previous application.
